Walk through any Malaysian night market and you will hear it: "Jangan makan durian, nanti darah tinggi!" Don't eat durian, your blood pressure will shoot up. It is one of the most repeated food warnings in Southeast Asia. But does a single durian sitting actually cause an immediate spike in blood pressure? The answer is more nuanced than the warning suggests.
What's Actually in Durian
Durian is energy-dense. Per 100g of flesh, it contains approximately:
- Calories: ~147 kcal
- Total carbohydrates: ~27g (including natural sugars and starch)
- Fiber: ~3.8g
- Potassium: ~436mg
- Fat: ~5g (mostly monounsaturated, with ~2g saturated)
A typical durian session of 3 to 5 medium pods delivers roughly 30 to 50g of carbohydrates. This is comparable to a bowl of rice or a couple of slices of bread — significant, but not extreme. Unlike soda, durian also brings fiber and fat that slow carbohydrate absorption.
The Blood Pressure Claim
Claim: Eating durian causes an instant blood pressure spike.
The reality: There is no clinical evidence that durian causes an acute, dangerous rise in blood pressure in healthy individuals. The carbohydrate load from durian triggers a normal insulin response, but blood glucose and blood pressure are different physiological systems. A sugar response does not automatically produce a blood pressure spike.
That said, durian can produce effects in specific situations:
1. Combined with alcohol Durian contains sulfur compounds that inhibit aldehyde dehydrogenase, the enzyme that breaks down acetaldehyde — a byproduct of alcohol metabolism. This interaction can cause flushing, nausea, and a temporary increase in heart rate, which some people misinterpret as a blood pressure crisis. The danger here is the alcohol combination, not durian alone.
2. Interaction with blood pressure medication Durian's potassium content (~436mg per 100g) can interact with certain medications, including ACE inhibitors and potassium-sparing diuretics. For people on these medications, a large durian intake could theoretically affect electrolyte balance. This is a medication interaction, not a direct effect of durian on blood pressure.
3. Excessive portion sizes Eating half a durian (500g+ of flesh) in one sitting delivers over 130g of carbohydrates and 700+ calories. Any large, heavy meal increases cardiac workload temporarily. A large plate of nasi lemak or a steak dinner does the same.
What the Research Shows
No peer-reviewed study has demonstrated that durian acutely raises blood pressure in healthy adults. The persistent belief likely comes from two sources:
- Durian's classification as a "heaty" food in traditional Chinese medicine
- Visible physical reactions after eating — flushing and sweating — which are thermogenic responses to digesting a calorie-dense food, not blood pressure events
Practical Takeaway
If you are a healthy adult not on blood pressure medication, eating 3 to 4 pods of durian is unlikely to cause any blood pressure event. The "instant spike" claim is not supported by evidence. However:
- Avoid combining durian with alcohol
- If you are on blood pressure medication (especially ACE inhibitors or potassium-sparing diuretics), consult your doctor about potassium intake
- Keep portions to 3–4 pods per session
- Stay well hydrated
